USE THIS PAGE TO SIGN UP FOR EVENTS & ACTIVITIES

1 Understand that you are welcome to come to anything just by showing up. We ask for people to sign up so we have an idea of how many people are coming - for things like transportation and food. There are some events that require registration - at this point, those registrations will all be handled with paper forms.

2 You will notice there are two ways to sign up for each event. Choose one that works for you - but only choose one! You don't need to register in both places (here and Facebook). Also - if you only want to sign up for one event now, do that. Then come back and complete the form for other events. You can fill this out as many times as you want.

3 Once you have signed up, please bring a check for the total amount of the trip to Jim. Checks should be made payable to Saint Matthew Lutheran Church.

Please have a parent or guardian review what you are signing up for and mark the check box below.

I represent that I am the parent/guardian of the above named child who is under 18 years of age.

I understand that the child named above, or I, will be participating in the event/events marked above.  I understand that during these activities my child/ward, or I, if I am an adult participant, will take part in a variety of activities consistent with the purposes of the St. Matthew Lutheran Church Teen Program.  I also understand that I can contact the leaders of this/these events for information on specific activities my child/ward will participate in.

I give permission for the child named above to participate in the activities of ST. MATTHEW LUTHERAN CHURCH, including any special events/activities marked above.  In consideration for allowing the participation of the child in the activities of ST. MATTHEW LUTHERAN CHURCH, I hereby consent to the Permission and Waiver Forms, including the Release of Liability clause, on behalf of the child and agree that these Permission and Waiver Forms shall be binding upon me, my family, heirs, legal representatives, successors, and assigns.

I need a copy of the Waiver & Medical Release Form (completed annually and kept on file).

Money owed for each event can be turned into Jim Bargmann at anytime after you sign up - even on the day we leave.
